Skeletal muscle alterations and post-exertional malaise in Long COVID and ME/CFS

Post-exertional malaise in Long COVID and ME/CFS reflect intrinsic biological dysfunction rather than simple deconditioning. Despite reduced aerobic capacity, patients show distinct skeletal muscle,
oxygen use and autonomic abnormalities, including mitochondrial dysfunction, altered fibre type, reduced capillary supply and persistently low heart rate variability. Exertion can worsen these systems, causing delayed recovery. There is a need for biomarkers, and wearables for personalizd care
